"IT IS HEREBY ORDERED that the attached forms "Exhibit A - Chapter 13 Plan Summary" and "Exhibit B -
Mandatory Form Chapter 13 Plan" and the instructions below are adopted by the Judge of this Court
for use by all Chapter 13 debtors for cases filed on or after August l, 2015. No other form of
